Please use this registration form for all campuses of DPC, Northeast Indiana. Note that all registrations are approved on a first-come, first-serve basis.

DPC registrations will not be accepted without subscribing to DASH. Please send DASH form and payment in the same envelope as your DPC registration form and payment, to the address listed on the DPC registration form.
